All students who wish to attend a term class at Canberra Potters with support from the NDIS must fill out this form.
Please note,
- 1 form must be filled out and submitted for each client.
- This form must be filled out prior to each Canberra Potters term.
- Completion of this form does not guarantee a place in a Canberra Potters class.
- Continuing students will have preference for class places.
- If you/your client is successfully placed in a class, you will receive an email confirming enrolment and class details.
- Term Class Fee - $490. This includes tuition, clay, glaze, firing and equipment/tools. Canberra Potters is not an NDIS registered organisation.
- We provide a single invoice for payment. We are unable to split invoices. Our terms are 14 days from date of invoice. All fees must be paid by the due date.

This form is for NDIS particiapnts wanting to attend group pottery sessions at Canberra Potters. Please note that Canberra Potters is not an NDIS provider. We can provide an invoice for particiapnts and/or their Plan Managers. We welcome your applications for next term.
